Service dog application
A service dog is individually trained to perform specific tasks related to a person’s disability. This differs from an emotional support dog, whose presence provides comfort but does not qualify it as a service dog under the Americans with Disabilities Act (ADA). A trained service dog generally may accompany its handler into places open to the public, including places where pets are not allowed. Please answer the questions below so we can learn about your needs and determine whether our Service Dog Program may be a good fit.
